Role Summary
As an Intake Coordinator, you will play a vital role following the agency’s policies and program frameworks by managing referrals for home health services. Your main task includes assessing the suitability of referrals and coordinating with professional field staff to ensure patient care aligns with physicians’ orders and payer authorizations, utilizing a collaborative system developed with the home health team.
Key Responsibilities
- Manage inquiry calls, verifying clinical eligibility and payer approval for services.
- Confirm Medicare and Medicaid eligibility for all referrals.
- Assess availability of staff and resources prior to accepting referrals.
- Ensure correct staff assignments and review orders for authorized medical services.
- Work in partnership with the RN Coordinator to handle intake documentation and care plans.
- Accurately record client demographics and communicate relevant information to caregivers.
- Participate in clinical rounds to stay informed about payer contract updates.
- Maintain clear communication channels with staff members and referral sources.
- Monitor admissions, current openings, and referrals not admitted, assisting in reporting efforts.
- Assist marketing initiatives and address any challenges related to referrals.
- Adhere to HIPAA standards to protect patient confidentiality.
- Inform caregivers when services resume and provide necessary clerical support.
- Engage in additional duties such as community outreach and ongoing education.
Qualifications and Skills
- Possess a high school diploma or GED; Medical Assistant or medical administrative experience is preferred.
- Experience handling admissions and staffing processes.
- Proficient computer and data entry abilities ensuring accuracy.
- Strong attention to detail with a focus on confidentiality.
- Excellent decision-making and problem-solving capacities.
- Effective oral and written communication skills.
- Superior organizational and time management skills.
- Capable of working independently and meeting deadlines.
