Role Overview
AdvisaCare is growing its home health and hospice team and is seeking a Home Health Intake Coordinator to support patient onboarding and intake operations. This position plays an important part in helping patients and families receive care promptly and accurately.
As the initial contact for patients and their families, you will collect key patient details, confirm insurance coverage, and guide the intake workflow from start to finish. Success in this role depends on strong attention to detail, excellent organization, and clear communication, along with a consistent focus on compliance.
What You Will Do
- Serve as the first point of contact for patients and family members during the intake process.
- Collect and record essential patient information accurately.
- Verify insurance details and coverage before care begins.
- Manage intake tasks efficiently to keep operations moving smoothly.
- Coordinate with healthcare professionals and families as needed.
- Maintain patient confidentiality and follow required protocols.
Requirements
- High school diploma or GED.
- Prior experience in home health care, insurance verification, or a similar role is preferred.
- Strong organizational skills and the ability to handle multiple tasks at once.
- Excellent verbal and written communication abilities.
- Working knowledge of electronic medical records systems is a plus, especially Wellsky, Kinnser, Allscripts, or EPIC.
- Comfort working collaboratively with healthcare teams and patient families.
- Detail-focused approach with a strong commitment to protecting patient privacy.
