About the Role
This position offers an early opportunity to help shape a growing care navigation program. As the Healthcare Intake Coordinator, you will be the first live point of contact for referred and prospective patients. You will make proactive phone outreach, explain the program in simple terms, complete clinical intake questionnaires accurately, and guide patients into active care without friction.
The role is highly metric-driven and centered on patient outreach. The ideal candidate combines empathy with steady follow-up, enjoys meaningful conversations with older adults, and understands that a completed intake means more than a verbal yes—it means the patient actually arrives for the first appointment.
The program is still being built, so workflows may change and priorities may shift. A person who enjoys problem-solving, collaborating, and working in an evolving environment will likely do well here.
Key Responsibilities
- Work through a structured outbound referral queue and contact prospective patients consistently, professionally, and with a warm approach.
- Explain the program clearly in patient-friendly language and capture standardized intake information during the conversation.
- Use empathy and strong communication to help older adults who may feel uncertain, hesitant, or confused about care navigation.
- Follow compliance steps carefully and collect all required verbal consents one by one before any scheduling takes place.
- Book the patient’s first clinical appointment immediately after intake is completed successfully.
- Record patient details, call outcomes, and clinical metrics accurately across multiple digital healthcare systems in real time while speaking with the patient.
Performance Expectations
- Maintain consistent daily outbound calling and contact activity.
- Improve the conversion rate from outreach contact to completed clinical intake.
- Help ensure patients attend their first scheduled assessment.
- Keep documentation accurate and follow all compliance checkpoints.
Requirements
- You must live in Alberta, Canada and be legally able to work without sponsorship.
- You should have a Medical Office Assistant certificate or diploma from an accredited post-secondary institution, or equivalent verified Canadian healthcare administration experience.
- You need at least 1 year of patient-facing healthcare experience, such as clinic front desk work, patient registration, care coordination, enrollment, or telehealth intake, with daily use of medical terminology and clinical data collection.
- Experience in a high-volume outbound calling environment is required, ideally in clinical outreach, patient scheduling, or health engagement work that required patient follow-through.
- You should be comfortable using multiple software tools and EHR systems at the same time while having an active, supportive conversation.
- A builder mindset is important: the role suits someone who can work through uncertainty, create structure, and adapt quickly as processes change.
- Prior remote work experience is strongly preferred.
