Role overview
Kent Community Health NHS Foundation Trust is seeking an experienced and enthusiastic Health Practitioner to step into a Team Lead position within the Health & Social Care Coordinator (HSCC) and Primary Care Lifestyle Teams in East Kent Long Term Services. This is a hands-on leadership role that combines clinical work with team support and service coordination.
The post is open only to colleagues currently employed within Kent and Medway NHS Trusts, the Kent and Medway Integrated Care Board (ICB), and Medway Community Healthcare. Nursing posts are also open to Student Nurses and Newly Qualified Nurses.
What the role involves
You will guide staff through supervision, mentoring, and line management while helping to run service delivery across both teams. At the same time, you will continue to work directly with patients, carrying out assessments, supporting care coordination, contributing to multidisciplinary team working, and directing people to the most suitable services.
The role also includes Trusted Assessor equipment assessments, with full training provided.
You will work closely with Community Nursing teams, GPs, and other partners to help reduce health inequalities, improve quality of life, and avoid unnecessary hospital admissions.
Leadership and service duties
- Support, mentor, and develop team members
- Oversee day-to-day delivery across both services
- Track resources and budgets, and contribute to service improvements
- Gather, review, and present data to help measure outcomes
- Promote safe, high-quality, evidence-based clinical practice
- Assess patient needs and arrange suitable referrals
- Communicate clearly with multidisciplinary teams and senior leaders
Location and working pattern
The post is based across Canterbury and Ashford, with regular travel for home visits, clinic appointments, and multidisciplinary meetings.
