Role Overview
The Health Engagement Specialist will focus on reaching out to Medicaid patients who require critical preventive care, including those who may excessively use emergency services or are transitioning from hospitals to home or alternative care venues. This position is perfect for empathetic, outgoing individuals eager to make a meaningful impact in patients’ healthcare journeys within a dynamic startup atmosphere.
Key Responsibilities
- Initiate a significant number of outbound communications to patients, many without prior interactions with Diverge Health.
- Handle incoming calls from patients responding to outreach or referrals, as well as from primary care providers coordinating care with Diverge Health.
- Identify patients frequently utilizing emergency rooms and guide them toward more suitable healthcare services.
- Engage patients during care transitions to ensure continuity, educate on preventive care importance, and promote routine provider visits.
- Assist patients by scheduling appointments, navigating healthcare options, and overcoming barriers to access.
- Build trust through tailored, empathetic interactions that honor individual patient needs.
- Document patient communications meticulously using CRM platforms such as Salesforce.
- Consistently meet or surpass daily outreach and engagement targets.
Qualifications and Skills
- 1 to 2 years of experience in healthcare patient engagement, outreach, or customer service roles.
- Excellent communication and interpersonal abilities to quickly establish rapport and trust.
- Compassionate attitude towards patients from various backgrounds.
- Capability to thrive in a rapidly changing work environment with shifting priorities.
- Strong analytical and problem-solving skills to address obstacles patients face securing care.
- Preferred familiarity with care coordination, preventive healthcare services, transitions of care, CRM tools like Salesforce, and call management systems.
- Effective organizational skills to juggle multiple assignments simultaneously.
