Role overview
AndHealth is working to expand access to high-quality specialty care for people managing difficult chronic conditions. The organization partners with health systems, community health centers, and independent specialists to remove care barriers and make treatment more affordable and accessible.
As a Health Coach, you will support patients in improving chronic disease outcomes by applying your background in nutrition and behavior change. You’ll collaborate closely with physicians, nurse practitioners, and patient success teams to help build and deliver individualized care plans. A background as a Registered Dietitian or Certified Nutrition Specialist is preferred.
This is a full-time, exempt position based in Columbus, Ohio.
What you will do
- Apply evidence-based coaching methods to strengthen patient engagement and improve outcomes.
- Help patients follow treatment plans created by healthcare providers while working with the care team to ensure coaching remains safe and effective.
- Partner with the broader care team to build integrated treatment approaches and support patients through coordinated coaching.
- Deliver integrated health coaching across a full patient panel, with attention to sleep, stress, nutrition, movement, and medication adherence, all within provider-directed care plans.
- Use behavior-change coaching techniques, including stages of change, to support patients over time.
- Gather, review, and monitor patient-reported outcomes and wearable-device data to shape individualized care plans alongside medical providers.
- Support medically necessary, personalized nutrition planning under the supervision of a registered dietitian to help reduce the severity and frequency of migraine attacks and autoimmune flares.
- Conduct virtual live sessions with patients to assess progress and adjust coaching approaches as needs evolve.
- Provide asynchronous, individualized coaching through technology platforms when needed.
- Work within clinical workflows using an EHR and other tools to document outreach, patient progress, and care activities accurately.
- Contribute ideas and feedback to improve internal processes and patient care workflows.
- Take on special projects as assigned.
Education and licensure
A bachelor’s degree is preferred. Completion of a health coach training program that makes you eligible to sit for the NBHWC certification exam is also preferred.
Experience and qualifications
- Prior experience supporting patients with chronic illnesses is required.
- Experience working in a medical setting or within an interdisciplinary care team is preferred.
- Comfort working with patient data and adapting to different technology platforms is important.
- You should be open to training in AndHealth’s integrative health coaching model and willing to follow the related protocols in patient care.
- A strong sense of accountability, initiative, and ownership for patient experience and outcomes is needed.
- You should be motivated to put the patient first and continue learning disease-specific knowledge to improve your effectiveness.
- Comfort with change and the ability to adapt quickly to evolving workflows is essential.
- Willingness to contribute beyond core job duties when needed is expected.
- You should care deeply about the quality of the work product and results you deliver.
- Strong collaboration and open communication across teams are important for this role.
- You should approach others with kindness, empathy, and anticipation of needs.
