About the role
The Health Coach supports people, their whānau, and the wider health team to strengthen wellbeing and improve health outcomes. The position is centred on helping clients grow their knowledge, confidence, and practical self-management skills so they can work toward their own health goals, make better use of health and social support, and take an active part in decisions about their care.
This role uses a holistic, culturally aware, and whānau-centred approach, with the aim of lifting wellbeing, equity, and overall outcomes for the communities served.
Key responsibilities
- Work with individuals and whānau to help them set and progress toward health goals.
- Link clients with relevant health, social, and community support services.
- Develop trusted relationships while providing culturally responsive guidance.
- Keep records accurate and contribute to reporting and service quality activities.
- Partner closely with whānau, community organisations, and the wider health team.

- Ability to speak Te Reo Māori and knowledge of tikanga Māori.
- Understanding of outcomes reporting or Results-Based Accountability (RBA).
