Overview

Unika Health, a subsidiary of iHealth Labs, offers an AI-driven chronic care platform led by physicians to assist healthcare providers in managing patients beyond traditional clinic visits. By integrating connected health devices, smart monitoring, and coordinated care teams, they enable continuous support for individuals with chronic conditions, bridging care between clinic appointments and daily life for improved outcomes.

Role Summary

The Certified Health Coach role focuses on overcoming barriers to evidence-based care like limited access, poor coordination, lack of patient engagement, and treatment adherence challenges. Working alongside a Dietitian and Clinical Assistant, the coach supports patients with hypertension, hypotension, diabetes, pre-diabetes, and obesity in a fast-paced, technology-reliant setting.

Key Responsibilities

  • Collect and review vital health data including blood pressure, glucose, weight trends, and dietary logs along with screening questionnaires.
  • Apply patient-centered coaching techniques to foster healthier habits, better outcomes, and enhanced quality of life.
  • Conduct in-person and virtual coaching sessions to help patients set and meet health and lifestyle goals with foundational education on chronic conditions.
  • Assist patients in overcoming barriers to goal achievement during and between visits through problem-solving support.
  • Maintain ethical standards and transparency to build trust with patients and the care team, even when extra effort is required.
  • Monitor patient vitals regularly to ensure stable clinical status and provide supplementary support as needed.
  • Ensure that care guidance complies with iHealth’s standards as well as recommendations from the American Heart Association (AHA) and American Diabetes Association (ADA).
  • Make outreach calls to patients with low digital literacy to guarantee equal educational opportunities and goal-setting support.
  • Follow up to confirm implementation of clinical and provider recommendations.
  • Escalate complex cases needing specialized nutrition interventions to Registered Dietitians or providers.
  • Support the Clinical Assistant in onboarding, data management, scheduling, patient screening, tech support, and engaging inactive patients.
  • Proactively identify and address patient needs, collaborating and delegating as appropriate to resolve issues efficiently.
  • Perform other duties as assigned by management.

Qualifications

  • Bachelor’s degree in Health Sciences, Nutrition, or related field, or an Associate’s degree plus minimum two years’ experience in health and wellness counseling.
  • If uncertified at hire, attain and sustain a nationally recognized health coach certification (e.g., NCC or NBC-HWC) within the first 12 months.
  • Bilingual proficiency in Cantonese, Tagalog, Vietnamese, Cambodian (Khmer), Russian, or Hmong is mandatory; Spanish or Mandarin proficiency is beneficial.
  • Experience in direct patient care including history-taking, vital assessments, goal-setting, and motivational interviewing.
  • Highly organized, technologically proficient, self-driven, and capable of managing time effectively in a dynamic environment.
  • Comfortable working independently and with culturally diverse patient and staff groups.
  • Excellent writing and verbal communication skills.
  • Familiarity with Google Suite applications such as Drive, Calendar, Docs, Slides, and Sheets.

Work Location and Compensation

This full-time hybrid/remote position is open to applicants residing in California, Texas, New York, New Jersey, Washington, or Virginia. The hourly wage ranges from $20 to $30, depending on location and experience.
