Job Summary
The Mental Health Care Coordinator collaborates directly with the Psychiatric Nurse Practitioner (APRN) and Psychiatrist to support clients receiving outpatient medication management. The role involves monitoring medication impact, coordinating overall care, and facilitating case and family meetings. This position emphasizes educating clients and their families or caregivers to foster self-sufficiency and enhance life quality.
Responsibilities
- Conducts initial and continuous assessments covering substance use, living conditions, support systems, mental status, client history, strengths, needs, and medical status.
- Supports and monitors mental health stabilization through frequent client interaction and evaluations.
- Coordinates treatment plan services, including community resource referrals, and evaluates their effectiveness and client satisfaction.
- Maintains consistent contact during medication appointments and as needed via telephone.
- Advocates for client rights in compliance with relevant state and federal regulations.
- Identifies and reports risks, trauma symptoms, and signs of abuse or neglect following established protocols.
- Keeps precise and timely records of all contacts, services, and communications.
- Develops and revises treatment plans with active client involvement, monitoring progress towards objectives.
- Ensures all required consent forms and releases of information are obtained and maintained.
- Utilizes Eleos Health, a HIPAA-compliant documentation tool, to optimize clinical documentation, allowing greater focus on client care. The tool converts session notes into comprehensive records, supports compliance, and offers insightful feedback accessible via desktop or mobile.
Qualifications
- A Bachelor's degree in Social Work or a related Human Services field (with a minimum of 30 credit hours studying human behavior and/or development) from an accredited institution is mandatory.
- Previous experience in human services or mental health is preferred.
- Must achieve proficiency with MHRC's Electronic Health Records system within three months of hire.
- Strong skills in Microsoft Office and email communication are required.
- Effective English communication skills, including clear, correct, and legible writing at a college graduate level, are essential for accurate record keeping and meeting quality standards.
- Exceptional customer service abilities to interact appropriately with clients, families, caregivers, community providers, supervisors, and professional staff.
Additional Details and Terms
- Work Schedule: Full-time, Monday to Friday, 8:00 AM to 4:30 PM.
