Job description

Role overview

This position supports adults who are eligible for Medi-Cal and are involved in the justice system through California’s Medicaid 1115 JI Reentry Initiative. The role focuses on outreach, care coordination, and non-clinical case management for people moving from state prisons or county jails back into the community. While clients are incarcerated, the Community Health Worker helps maintain Medi-Cal coverage and connects them with health care, medications, Enhanced Care Management (ECM), and community support services. The work is centered on improving reentry success, strengthening health outcomes, and helping reduce repeat justice involvement through respectful, culturally aware, client-centered support.

Key duties

  • Deliver release-planning support for clients with a forensic background, mental health needs, and/or dual diagnoses.
  • Review incoming referrals and determine the release-planning needs tied to each case.
  • Meet with clients to explain available services, work through resources and the Initial Release Plan (IRP) for the relevant service area, and support their participation in release planning.
  • Use different databases and applications to look up client history, current location, court dates, housing details, health information, and level of need.
  • Record client interactions in the medical chart without delay.
  • Prepare internal paperwork and reports in a timely way.
  • Support training and onboarding efforts for PRCM staff as program needs require.
  • Complete DHS-CHS department trainings on schedule.
  • Coordinate with community partners, county contacts, and internal teams so clients can access the services and resources they need.
  • Take on additional responsibilities as assigned.

Qualifications

Candidates should be able to follow both spoken and written directions, use good judgment in a high-stress setting, and meet the physical demands of the job, including lifting up to 10 pounds and doing some bending, stooping, or squatting.

Experience and background

At least 6 months of full-time experience is required in a public-facing or community-based role, such as interviewing clients or patients about health or social service concerns, answering questions, and sharing information about health, mental health, and social services.

Clearances and requirements

  • Must successfully complete the LA County Live Scan process.
  • Must pass the required health clearance.
  • Must be willing to work in correctional health facilities.

Physical requirements

The role involves frequent standing, walking, and sitting. Handling/fingering, reaching outward, reaching above the shoulder, climbing, crawling, kneeling, and bending are occasional. Lifting, carrying, pushing, and pulling are occasional and limited to up to 10 lbs. Seeing is a constant requirement, while taste/smell is not applicable. Frequency definitions: occasionally means 0 to 2 hours per day, frequently means 2 to 5 hours per day, and constantly means 5+ hours per day.

Work environment

General office setting with indoor, temperature-controlled conditions.

Compensation

The hourly pay range for this role is $23.00 to $24.88.
