Clinical Pharmacist, Population Health (Remote)

Overview

This full-time remote role involves independent review, preparation, and dispensing of medications according to physician orders and institutional protocols. The pharmacist ensures that patients receive safe, effective, and economical therapies while adhering to departmental guidelines.

Key Responsibilities

• Review and dispense medication orders following established pharmacy procedures.
• Provide clinical advice and clarify medication therapies to healthcare practitioners, including suggesting cost-effective alternatives.
• Offer drug information and counseling services to patients, families, and healthcare teams.
• Support training, education, and assessment of pharmacy students and residents.
• Supervise pharmacy support staff and oversee technician daily activities.
• Design, coordinate, and manage programs that enhance cost and quality outcomes in value-based care populations, focusing on medication and disease management.
• Use data analytics to identify patients for intervention and collaborate with providers and care teams to implement effective drug therapy plans.
• Apply expertise in care coordination, population health, value-based contracting, and clinical program development.

Qualifications and Requirements

• Bachelor of Science in Pharmacy from an accredited institution (minimum five years).
• Current registered pharmacist license and controlled substance license issued by the Michigan State Board of Pharmacy.
• Basic Cardiac Life Support (BCLS) certification may be required.
• Preferred advanced clinical training in a specialized area.
• Previous experience in hospital or retail pharmacy is advantageous.
• Proficient in evaluating drug regimens, interacting with pharmacy software for order entry, allergy screening, and drug interaction checks.
• Excellent verbal and written communication skills, with basic familiarity in Microsoft Word, Excel, and other computer applications.
• Comfortable using standard desktop and Windows-based systems, including email, e-learning platforms, and intranet navigation.
• Physical ability to perform repetitive manual tasks, lift light weights (up to 40 lbs), and spend prolonged periods sitting or standing.

The position offers a salary range of $110,000 to $130,000 per year and involves a day shift schedule.
