Overview

The Clinical Behavioral Health Coordinator is responsible for managing behavioral health services coordination, supporting treatment plan development, overseeing quality outcomes, and supervising Care and Resource Coordinators. Serving as the central communication link among participants, behavioral health providers, community partners, and internal staff, this role ensures coordinated, person-centered, and effective care delivery.

Key Responsibilities

  • Coordinate care among participants, providers, support teams, and community resources to foster integrated service delivery.
  • Facilitate communication between behavioral health providers and community partners.
  • Assist participants in accessing appropriate behavioral health, medical, social, and community services.
  • Develop collaborative partnerships with external providers and agencies.
  • Conduct regular reviews of participant services aligning with treatment goals and addressing barriers to care.
  • Maintain knowledge of behavioral health programs, regulations, and licensing requirements.
  • Monitor program performance, conduct quality improvement initiatives, and engage in program evaluation.
  • Create and monitor treatment plans, ensuring compliance and interdisciplinary collaboration.
  • Supervise Care Coordinators and Resource Coordinators, providing coaching, mentoring, and fostering professional growth.
  • Manage accurate data records, generate reports, and support audits and accreditation activities.
  • Promote person-centered, recovery-oriented, and trauma-informed care while maintaining confidentiality and ethical standards.
  • Adhere to organizational bylaws, policies, OSHA, HIPAA, and safety procedures.

Qualifications

  • High School Diploma or GED required; Bachelor's degree in Human Services or related field preferred.
  • Minimum 3 years relevant experience in behavioral health, case management, or care coordination.
  • Preferred 2 years' experience with behavioral health populations including mental health, intellectual/developmental disabilities, and substance use disorder treatment.

Skills and Competencies

  • Excellent verbal and written communication skills.
  • Strong organizational, documentation, analytical, and critical thinking abilities.
  • Effective multi-service coordination and leadership capabilities.
  • Proficiency with Microsoft Office and electronic health record systems.
  • Ability to maintain confidentiality and professionalism in a fast-paced environment.
  • Strong interpersonal skills with commitment to cultural humility and collaboration.

Additional Information

  • Must hold a valid driver's license, current vehicle registration and insurance, and reliable transportation for travel within the community.
  • Physical requirements include prolonged sitting, standing, walking, lifting up to 50 pounds, and travel across multiple office and healthcare settings.
  • Work schedule flexibility may be necessary, including varied hours, travel, evenings, and weekends.
  • Work in a drug-free workplace with required testing under applicable circumstances.
  • Maintains professionalism, ethical conduct, and sound judgment, supporting organizational quality initiatives.
