Role overview

This position focuses on improving outcomes for older people in Western Sydney by carrying out comprehensive aged care assessments and linking individuals with the right support at the right time. The role is part of a multidisciplinary service based at Westmead and involves working closely with patients, families, carers, health professionals and external providers.

What you'll do

You will assess older people, build practical support plans and help them access Commonwealth-funded aged care services, residential care pathways and community supports. The role requires a person-centred approach that promotes independence, positive ageing and quality of life.

Employment details

This is a temporary full-time opportunity for 4 positions, offered as a 12-month contract up to 30 June 2027. Standard hours are 38 per week, and job sharing may be considered for part-time arrangements. The role is classified for Registered Nurses and Allied Health Professionals at Level 1/2.

Remuneration is dependent on qualifications and ranges from $76,364 to $111,427 per annum, plus superannuation.

The Seniors Assessment Service is committed to helping older people receive appropriate care at the right stage of their journey. You will be part of a skilled team delivering aged care assessments across both hospital and community settings. The environment is collaborative, diverse and focused on continuous improvement.

Key accountabilities

• Carry out holistic assessments covering medical, functional, cognitive, psychosocial, cultural and environmental factors.
• Create evidence-based support plans and support access to residential aged care and community services in line with the Aged Care Act.
• Work with multidisciplinary colleagues and external agencies to ensure coordinated, person-centred care.
• Maintain precise clinical records and handle assessment information according to legal and organisational requirements.
• Respond to referrals promptly while meeting service targets and maintaining assessment quality.

Essential criteria

• A tertiary qualification and current AHPRA registration in Nursing, Physiotherapy or Occupational Therapy; or a tertiary qualification in Social Work with eligibility for membership of the Australian Association of Social Work; or Speech Pathology with eligibility for membership of Speech Pathology Australia.

Selection criteria

• Postgraduate experience working with older people in either acute or community environments.
• Ability to work independently with initiative, strong organisation, sound problem-solving and effective time management.
• Strong written and verbal communication skills, along with solid computer literacy.
• Capacity to work alone and also contribute effectively within a multidisciplinary team.
• Knowledge of, or experience with, the health and functional support needs of older people and their carers in the community.
• A current Australian Class C driver licence and proven competent driving ability.
