Role overview
This position sits within a committed Children in Care team as part of the wider 0-19 Service. The post involves supporting the health and wellbeing of children and young people who are in care, with a focus on delivering comprehensive and person-centred nursing support.
Key duties
You will assess, plan and carry out holistic health care plans, with support delivered in schools, homes, clinics and other relevant settings. The role includes completing statutory health assessments and encouraging better health outcomes for children and young people.
- Build and maintain effective working relationships with young people, parents or carers, social care professionals and colleagues across voluntary and statutory services.
- Handle sensitive disclosures carefully, share concerns with the right people or agencies, and help ensure children and young people remain safeguarded.
- Maintain clear, accurate and current paper and electronic records, and prepare written reports when requested by service leadership.
- Work with colleagues and managers using a variety of communication channels, including digital communication tools.
- Assess needs, plan care and deliver interventions and programmes for young people in the care system up to the age of 25.
- Identify health and wider social needs affecting care leavers and other young people in this cohort.
- Spot signs that may indicate child or adult abuse and follow safeguarding procedures in line with CCS policies and the relevant safeguarding boards.
- Carry out community visits, which may sometimes be outside standard working hours, in accordance with the Lone Workers policy.
