About the Role

We are looking for a committed Case Manager to oversee care coordination within behavioral health services, focusing on substance use disorder (SUD) and mental health (MH) patient populations. This role requires direct involvement in multiple facets of client management, including facilitating group sessions and managing communication across various care facilities.

Primary Responsibilities

  • Act as the primary liaison for clients across several treatment locations to ensure smooth communication and service integration.
  • Lead psychoeducational and support groups, with a preference for candidates having prior group facilitation experience.
  • Assess client needs, recognize obstacles, and connect individuals with suitable resources and support.
  • Organize provider appointments and maintain thorough documentation within electronic medical records (EMR).
  • Liaise with external parties such as probation officers, employers, and legal professionals.
  • Assist clients with admission processes, discharge planning, and aftercare arrangements to ensure continuity of care.
  • Monitor client participation, attendance, progress, and adherence to programs, and escalate concerns as appropriate.

Candidate Qualifications

  • Between one and three years or more of experience in behavioral health care coordination or case management, ideally with exposure to SUD and MH populations.
  • Preferable experience facilitating group sessions.
  • Knowledge of insurance payer processes, compliance standards, and available community resources.
  • Educational background typically includes a Bachelor's degree in social work, psychology, or a related discipline, or equivalent practical experience.
  • Certifications relevant to the field are advantageous but not mandatory.
