Job Summary
The Care Navigator will be essential in assessing and planning interventions to support clients facing social, emotional, economic, and environmental challenges. This role entails facilitating access to community resources, assisting long-term care insurance policyholders in their care journeys, and contributing to the growth of the care management program.
Key Responsibilities
- Engage clients and their families to understand caregiving challenges and identify obstacles to successful aging in place using phone and email communication.
- Utilize motivational interviewing techniques to effectively connect with individuals having diverse health needs.
- Maintain accurate and thorough documentation of communications and interventions.
- Act as a subject matter expert in care management, care coordination, and resource navigation.
- Collaborate with the development team to enhance care management tools and systems.
- Mentor and support new or junior team members.
- Manage caseloads efficiently and meet outreach goals for client touchpoints.
- Perform additional duties as assigned.
Performance Metrics
- Complete 90% of needs assessments timely.
- Manage caseloads effectively to ensure timely handling of cases.
- Complete outbound client touchpoints within service level agreements.
- Ensure documentation reviews are accurate and comprehensive.
Required Skills & Abilities

- Proficiency in multi-channel client communication (phone, email).

- Must be at least 18 years old.
Education & Experience
- Strong practical experience in geriatric case management or coordination.
- Preferred expertise in social services, care coordination, healthcare navigation, public health, community organizing, or volunteering.
- At least 1 year in a customer service or comparable role preferred.
- Experience in high-volume call centers is a plus.
- Bachelor’s degree in Psychology or Social Work is preferred.
Physical and Working Conditions
- Capability to remain seated at a workstation during the workday.
- Daily use of computers, phones, and VoIP technology.
- Strong communication skills to clearly exchange information.
- Ability to observe details closely on computer screens.
- Remote work from approved U.S. locations, requiring reliable internet and a professional workspace.
