Job Summary
As a Care Management Scheduler, you will contact eligible patients, explain how care management programs can help them manage chronic conditions, confirm coverage details, and keep patient records accurate. The role requires clear, compassionate communication and close coordination with internal teams and healthcare providers to ensure patients receive consistent support.
Key Responsibilities
- Reach out to eligible patients and walk them through the value of care programs designed for chronic condition management
- Check patient eligibility and verify insurance information
- Keep documentation of patient conversations current, accurate, and complete
- Communicate in a polished, empathetic manner that helps establish patient trust
- Work with internal departments and healthcare providers to support smooth patient service delivery
Required Qualifications
- Previous experience in a healthcare environment is preferred
- Comfort using basic computer tools and performing data entry tasks
- Strong troubleshooting ability and the capacity to juggle multiple tasks
- Excellent spoken and written communication skills
- Ability to work independently in a remote setting
