Job description
About Triplemoon
Triplemoon is a digital health company focused on improving family wellness by closing important gaps in mental, behavioral, and nutritional care. The organization works with providers and employers to give families easier access to evidence-based mental health support that can improve outcomes, expand provider capacity, and strengthen clinic financial performance. As demand for personalized mental health support continues to rise and provider availability remains limited, Triplemoon aims to address a major care gap for families and clinics. The company is expanding its collaborative care services and is growing its team.
Position Summary
The Behavioral Health Team Lead is a hands-on clinical leadership position responsible for guiding and supporting the day-to-day work of Triplemoon's Behavioral Health Care Manager (BHCM) team. Reporting to the Director of Behavioral Health, this role sits between clinical quality and team operations, helping ensure that BHCMs provide compliant, high-quality, and clinically appropriate care within the CoCM program.
This opportunity is ideal for an experienced clinician with strong leadership instincts, a sharp eye for quality, and the ability to keep a distributed team organized and moving efficiently. It is especially suitable for a BHCM who is ready to step into a formal leadership role.
Key Responsibilities
- Act as the main day-to-day contact for BHCMs with operational and care-delivery questions.
- Track team schedules, case distribution, and daily workflows to maintain steady patient coverage.
- Guide BHCMs through challenging patient situations and escalate concerns to the Director of Behavioral Health and Chief Clinical Officer when needed.
- Help build a team environment grounded in accountability, clinical quality, and psychological safety.
- Hold regular one-on-one check-ins with BHCMs to identify obstacles and support morale.
- Run routine shadowing sessions with BHCMs and give structured, real-time feedback on clinical performance and documentation.
- Perform chart reviews to evaluate documentation quality, care cadence compliance, and registry accuracy.
- Spot recurring quality issues and raise broader concerns to the Director of Behavioral Health.
- Support the rollout and adoption of updated workflows and SOPs created by the Manager of Education, Curriculum & Credentialing.
- Participate in weekly registry review meetings led by the Sr. Manager of Psychiatry.
- Document action items from registry review and follow up with BHCMs to ensure timely completion.
- Monitor patient engagement against clinical cadence standards and flag unusual cases for registry review discussion.
- Serve as an operational link between the BHCM team and clinical leadership, surfacing workflow blockers and improvement opportunities.
- Help onboard new BHCMs in partnership with the Director of Behavioral Health and the Manager of Education, Curriculum & Credentialing.
- Contribute to the refinement of clinical operating procedures based on team-level experience and feedback.
- Assist with tracking BHCM performance metrics and support the Director of Behavioral Health with performance management input.
Qualifications
- Active clinical license such as LCSW, LPC, LMFT, or an equivalent credential in at least one state supported by Triplemoon.
- At least 3 years of direct clinical experience in a behavioral health setting.
- Background in a Collaborative Care Model (CoCM) or integrated behavioral health environment is strongly preferred.
- Proven ability to coach peers, provide feedback, or offer informal leadership to clinical colleagues.
- Strong organizational abilities and comfort handling multiple priorities in a fast-moving remote setting.
- Experience using EHR systems and registry-based care management workflows.
- Preferred experience in a supervisory, lead, or team coordination role.
- Knowledge of CMS documentation standards and billing-eligible encounter requirements under CoCM is preferred.
- Multi-state licensure, or a willingness to obtain licensure in Triplemoon's supported states, is preferred.
- Experience using tools such as PHQ, GAD-7, PSC-17, or similar standardized behavioral health screeners is preferred.
What Success Looks Like
During the first 90 days, the Team Lead is expected to become well-versed in BHCM workflows, begin shadow sessions and chart reviews, and take ownership of the weekly registry review list. By the six-month mark, the person in this role should be a reliable operational partner for the BHCM team, with clear progress in care cadence adherence, documentation quality, and team engagement.
Compensation
The salary for this remote role is USD 95,000 to USD 105,000 per year.
