Role overview
The Community Independence Service (CIS) is looking for a Band 6 Physiotherapist to join its rehabilitation team in a role focused on helping older people remain well at home and avoid unnecessary hospital admission. This service supports residents of Hammersmith & Fulham and works across the inner London boroughs as part of a joint arrangement between West London NHS Trust and CNWL Foundation Trust.
In this post, you will deliver an integrated health and social care service for people who are at risk of hospital admission. The work includes community rehabilitation, rapid response, reablement and supported discharge, with close collaboration with acute hospital partners such as Imperial College Healthcare NHS Trust and Chelsea & Westminster Hospital NHS Foundation Trust.
You will also connect with colleagues across Hammersmith & Fulham, Westminster, and Kensington & Chelsea, contributing to service improvement work and benefiting from peer support across the tri-borough area. The Trust will support your clinical and leadership development in line with your personal development and appraisal plan, and you will be set up for mobile working with flexible working arrangements where appropriate to the needs of the service.
Main duties
Your work will focus on delivering rehabilitation and preventative interventions for people receiving care at home. You will play a key role in supporting a client group where your contribution can make a meaningful difference every day, while working within a multi-agency environment that encourages learning, teamwork and shared support.
Service and organisation information
The Community Independence Service provides planned and unplanned nursing and therapy support for older adults who have experienced a recent decline in physical health. West London NHS Trust works jointly with CNWL Foundation Trust in this service across three inner London boroughs and is part of a wider community independence offer across Westminster and the Royal Borough of Kensington & Chelsea.
