Associate Care Advocate - Care Delivery

Role Summary

This entry-level Associate Care Advocate position supports patients by uncovering health-related obstacles and directing them to the right community-based resources. The role is an important part of driving patient engagement and helping people access the services they need. It also offers a path for growth into more advanced care delivery roles.

What You Will Do

• Use a scripted phone questionnaire to identify patient concerns and needs.
• Establish rapport so patients feel comfortable sharing information openly.
• Guide patients toward appropriate health, mental health, and social support services.
• Record patient details and consent accurately in the electronic health record system.
• Follow federal privacy rules and billing-related compliance requirements.
• Handle daily scheduling, send reminders, and arrange follow-up calls.
• Work toward weekly performance targets.

What We Are Looking For

• High school diploma or GED is required; a degree in Human Services, Public Health, or a similar area is preferred.
• Fluency in English and Spanish, or another second language, is strongly preferred.
• Community Health Worker certification is preferred, or the willingness to earn it through training.
• Comfort speaking with diverse groups over the phone.
• Basic comfort using websites and data-entry tools.
• Ability to work independently with limited supervision.
• Completion of onboarding training, including call practice and documentation exercises, is required.
• Must be able to pass a background check and drug screening.
• Must be physically able to sit, stand, walk as needed, and use computer equipment.
• Reliable internet access and a quiet, private workspace are required for calls.

Compensation

The pay range is $18 to $20 per hour, with performance-based bonuses available. The role also includes a clear progression path to more senior Care Advocate positions.
