Southern Cross Care WA is seeking a Physiotherapist to join its Home Care team in Rivervale, Western Australia. This role sits within one of WA’s major aged care providers and is focused on delivering person-centred support that helps clients improve their independence, wellbeing and day-to-day function.

The successful candidate will use advanced clinical judgement to assess clients, plan evidence-based care and work closely with multidisciplinary colleagues to support client goals.

Key duties

You will deliver physiotherapy services that are outcome-focused and tailored to client needs. The work includes assessment, clinical reasoning and intervention across a variety of referral pathways, with an emphasis on functional improvement and quality of life.

• Provide evidence-based physiotherapy care with measurable outcomes.
• Support acute and ongoing pain management.
• Deliver functional reablement and goal-based rehabilitation.
• Run falls prevention and recovery programs.
• Assist with the management of chronic disease and multimorbidity.
• Assess and prescribe mobility aids where appropriate.
• Communicate and coordinate with the broader multidisciplinary care team.
• Contribute to continuous quality improvement activities.

Candidate profile

The ideal applicant is a senior physiotherapist with strong experience, a genuine commitment to working with older people and the ability to contribute independently as well as collaboratively. A person-centred approach and a desire to improve clients’ lives are essential.

To be considered, you should hold current AHPRA registration as a Physiotherapist with general registration and no practice-relevant restrictions or conditions. The role calls for 3-5+ years of post-graduate experience, along with confident senior-level assessment, evidence-based intervention and clinical reasoning skills.

Applicants must be able to meet pre-employment and ongoing compliance requirements. If shortlisted, you may be asked to provide a current National Police Clearance issued within the last 6 months (or proof that it has been applied for), an NDIS Worker Screening Check, an influenza immunisation record, a completed pre-employment health declaration and evidence of working rights.
