Role overview
Hume City Council is seeking a Care Partner to join its Community Health & Wellbeing team in Broadmeadows, Victoria. This is a fixed-term full-time role for 18 months, offering an annual salary of $99,526.96 plus superannuation.
The council is one of Australia’s fastest growing and most culturally diverse local governments, with a strong focus on putting the community and customer at the centre of service delivery. In this position, you will provide holistic case management, practical assistance, and tailored support to clients in line with the objectives and standards of the Support at Home program.
Key duties
- Offer client-focused guidance and support to help people understand the aged care system and make choices that suit their needs.
- Work with clients to identify and reassess goals, then build service plans that fit within their Support at Home budget.
- Arrange and oversee personalised support arrangements that help clients remain safely in their homes and stay connected to their communities.
- Maintain accurate client records and documentation in accordance with internal procedures, policy requirements, and relevant legislation, with a strong focus on compliance and data accuracy.
- Apply knowledge of key legislation, including the Aged Care Act, the Aged Care Quality and Safety Commission Act, the Privacy Act, and Freedom of Information legislation.
- Contribute actively to continuous quality improvement initiatives within Aged & Disability Services.
- Support occupational health and safety measures that protect clients, employees, and service providers.
- Take part in developing, implementing, reviewing, and evaluating policies and procedures that strengthen service delivery.
Experience and capabilities
- Sound understanding of the Aged Care Quality Standards and how to apply them in practice.
- Strong background in assessment and case management for clients with high needs and complex circumstances.
- Ability to communicate and collaborate effectively with medical practitioners, allied health professionals, and other community stakeholders.
- Capacity to build constructive relationships with clients, providers, and the wider community.
- Confident use of computers and client management systems.
- Practical commitment to continuous quality improvement, workplace safety, and person-centred care.
- Strong written and verbal communication skills, including report writing, correspondence, rapport-building, and prompt, respectful issue resolution.
- Knowledge of occupational health and safety principles and the risks associated with home care services.
- Experience in person-centred case management and understanding of consumer-directed care package principles.
Qualifications and checks
- A Certificate IV, Diploma, or tertiary qualification in Allied Health, Community Services, Nursing, or a related field, together with relevant experience.
- A current driver’s licence is required.
- Successful candidates must complete background and probity checks, including reference checks, a Working with Children Check, and a criminal record check.
