Healthcare Support Worker

Job Overview

An opportunity is available for a skilled Healthcare Support Worker at Sittingbourne Memorial Hospital within Kent and Medway Mental Health NHS Trust. This role focuses on collaborating with service users to foster engagement and enable greater independence and self-management, within the context of mental health service transformation under the community mental health framework.

Key Responsibilities

• Monitor both physical and mental health outcomes of clinic attendees.
• Assist in conducting physical health assessments and relevant interventions to improve the wellbeing of service users with mental health conditions.
• Support primarily individuals with severe, long-term mental illnesses such as psychosis.
• Collaborate closely with registered mental health nurses, general nurses, pharmacy technicians, and pharmacists in the clinic team.
• Contribute to developing and monitoring mental and physical health care plans, assessing health and social outcomes for clinic clients.
• Promote positive physical and mental health awareness among service users, carers, families, and staff members.
• Operate medical devices for monitoring purposes, accurately recording readings with an understanding of normal and abnormal values.
• Perform blood sampling as part of physical health monitoring duties.
• Participate in routine completion of clinical data entry on IT systems.
• Encourage service users to consider their health needs and assist in identifying and tracking their personal health goals alongside the healthcare team.
