Position Overview

The Remote Care Coordinator plays a vital role within a multidisciplinary team by ensuring timely coordination of healthcare services tailored to meet individual patient needs. This role supports high-quality clinical outcomes and promotes an excellent visit experience.

Responsibilities

• Initiate proactive outreach to patients for tracking lab tests, imaging, referrals, and addressing care gaps to improve quality measures.
• Handle administrative functions including scheduling appointments, managing lab and imaging orders, assisting with pharmacy prescriptions and calls, completing referrals, updating electronic health records, managing insurance paperwork, and processing payments.
• Communicate empathetically and professionally across multiple channels such as phone, video calls, email, messaging platforms, and patient portals.
• Provide effective patient communication to plan, implement, and follow-up on healthcare needs.
• Offer real-time clinical support for troubleshooting remote healthcare service challenges and assisting patients with lab, imaging, or pharmacy concerns.
• Identify patient issues and refer to providers as appropriate, escalating urgent matters to practice leadership promptly, such as adverse events or patient complaints.
• Ensure compliance with HIPAA and internal policies to safeguard patient privacy and safety.

Requirements

• Medical Assistant certification is mandatory.
• 2 to 3 years of customer service experience in healthcare environments that use hybrid care models.
• Experience working with CoverMyMeds system is required.
• Proficiency in navigating and using electronic medical records (EMR).
• Flexibility to work nights or weekends across different US time zones (Pacific and Eastern).
• Highly organized, detail-focused, and self-motivated individual.
• Availability to work Tuesday through Saturday, from 7:00 am to 4:00 pm PST or 10:00 am to 7:00 pm EST.

Compensation and Benefits

• Hourly wage range: $18 to $22.

Additional Information

The position includes a 90-day probation period. Due to the nature of this role, employees will handle sensitive patient data regulated by HIPAA; comprehensive privacy and security training will be provided.
