Position Overview
The Care Navigator role, based in Greater Los Angeles with a focus on skilled Pediatric Day Nursing (PDN), involves partnering with location leadership to promote Aveanna Healthcare’s services through strategic business development. This role entails establishing and maintaining strong referral networks with healthcare providers including physicians, hospitals, specialty clinics, and social workers.
Key Responsibilities
- Develop and nurture relationships and increase brand awareness among referral sources and community stakeholders.
- Conduct sales activities, communications, and support marketing efforts within the assigned territory.
- Act as a public representative to ensure exceptional customer service with referral partners.
- Collaborate with leadership to create sales strategies, set goals, and assess sales performance.
- Participate in community education events related to medically fragile pediatric populations.
- Maintain regular contacts with health professionals to strengthen trust and understanding of Thrive SPC services.
- Monitor and enhance referral source satisfaction, providing feedback and solutions as needed.
- Meet or surpass quarterly and annual sales targets.
- Submit timely documentation including sales call records, contact information, account profiles, and expense reports.
- Fulfill other assigned duties.
Qualifications
- Bachelor’s degree or equivalent experience in a related field.
- At least one year of experience in sales, marketing, customer service or B2B environments.
- Self-driven with a proven record of achieving results.
- Excellent verbal, written communication, and interpersonal skills.
- Proficient in Microsoft Office applications (Word, Excel, Outlook, PowerPoint).

Additional Requirements
- Extensive travel within the designated territory and attendance at team or company meetings is mandatory.
- Physical demands include regular visual and auditory attention, prolonged sitting, keyboard and mouse usage, occasional standing, walking, bending, kneeling, and light lifting up to 10 pounds.
- Potential exposure to blood or body fluids may occur occasionally, classified as Risk Category II.
