Role Summary
The Nurse Navigator will facilitate coordinated care across the health system, aiding patients by addressing access issues, connecting them with resources, providing educational materials, and fostering relationships with service providers and community entities. This role focuses on timely and efficient care coordination, identifying and overcoming barriers, and supporting patients throughout their care journey. The Nurse Navigator also evaluates the physical, psychological, and social needs of patients to improve outcomes, boost patient satisfaction, and lower care costs. Additionally, the role includes responsibilities in community education, screening, outreach, and survivorship promotion.
Primary Workplace and Department
- Department: Physical Rehabilitation
- Facility: Resurrection Medical Center
- Location: 7435 W Talcott Ave, Chicago, IL 60631
- Work schedule as needed (PRN) with particular emphasis on weekends
Qualifications and Certifications
- Must hold a current, valid Registered Nurse (RN) license within the state
- Maintain a current Basic Life Support (BLS) certification upon hire and throughout employment
- Bachelor of Science in Nursing (BSN) is preferred
- At least two years of nursing experience required
Compensation and Benefits
This position offers a competitive compensation rate estimated at a flat $43.09 per hour. Final salary will be determined based on relevant experience, skills, education, credentials, and licensure during candidate selection.
