HCC Coder and Quality Assurance Specialist

About the Role

We are seeking skilled HCC Coders and QA professionals to join our team in Chennai. The ideal candidates will review medical records to identify patient diagnoses and treatments, assign appropriate ICD-10-CM diagnosis codes, and map these to Hierarchical Condition Categories (HCC). Key responsibilities include maintaining coding accuracy and compliance, auditing clinical documentation, and providing constructive feedback to healthcare providers. Additionally, the role involves educating physicians and clinic staff on clinical documentation standards to improve compliance.

Key Responsibilities

• Analyze medical records to determine patient diagnoses and treatments.
• Accurately assign ICD-10-CM codes and map diagnoses to HCC categories.
• Ensure consistency and correctness in code assignment.
• Conduct audits to verify documentation and coding accuracy.
• Provide feedback to physicians and providers regarding coding practices.
• Support education efforts for clinical documentation compliance among healthcare staff.

Required Skills and Qualifications

• Demonstrated experience in medical coding with strong attention to detail.
• Commitment to maintaining coding accuracy and regulatory compliance.
• Proficiency with ICD-10-CM, CPT, HCPCS, and HEDIS CAT II codes.
• Possession of at least one certification among CRC, CPC, or CCS is mandatory.
• Minimum experience: Non-certified HCC Coders with at least 9 months, certified coders with 1+ year, and QA professionals with over 2 years.
