Role Summary
As a Care Navigator on the Care Delivery Operations team, you will engage with patients primarily via phone to conduct health assessments and initial intake appointments. This entry-level role focuses on establishing rapport quickly, understanding social determinants affecting health such as food, transportation, and housing, and maintaining meticulous records of every interaction. This position offers a clear advancement path to become a Care Advocate.
Responsibilities
- Conduct health screenings and intake calls using a standard question set, establishing trust to gather accurate patient information during 15-20 minute conversations.
- Manage scheduling by booking intake appointments directly in the Healthie system; maintain continuity by personally scheduling follow-up appointments rather than transferring patients.
- Follow up to confirm appointments, aiming to achieve a show rate of 60% or higher.
- Identify patients' needs through open-ended questions and arrange appropriate next steps, such as doctor visits or meetings with Community Health Workers.
- Accurately document patient details in real-time while adhering to HIPAA regulations, ensuring all consent forms and notes are completed before shift end.
- Organize daily tasks effectively to meet set goals and maintain thorough documentation.
Requirements
- Minimum educational qualification: High school diploma or its equivalent; degrees in Human Services, Public Health, or Behavioral Health are advantageous.
- Preferred bilingual fluency in English and Spanish or an alternative second language.
- No previous intake experience necessary; however, familiarity with healthcare, social services, community outreach, or platforms like Healthie is beneficial.
- Comfortable using digital and web-based applications, adaptable to dynamic work environments.
- Ability to work independently with minimal supervision, maintaining organization and meeting targets while working remotely.
- Willingness to complete comprehensive training including call simulations and documentation practices, passing final assessments with at least 80%.
- Must clear a background check and drug test prior to employment.
Physical and Work Environment Requirements
- Capable of sitting, standing, and walking as necessary to perform duties effectively.
- Good hearing and vision to communicate clearly with patients both over the phone and in person.
- Adequate computer skills to update health records while maintaining conversational focus.
- Reliable internet connection and a private, quiet space required for remote interactions.
