Position Overview
We are looking to hire a qualified Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our home care team. The selected professional will provide high-quality nursing care to patients in their homes, collaborating closely with families and other healthcare providers to promote the best possible patient outcomes.
Key Responsibilities
- Evaluate patients' health status and create personalized care plans
- Administer prescribed medications, injections, and treatments
- Monitor vital signs and keep accurate records of patient progress
- Provide wound care, catheter maintenance, and assorted medical procedures
- Educate patients and their families on managing diseases and sustaining health
- Coordinate care efforts with physicians, therapists, and other health professionals
- Ensure documentation complies with regulations and is timely
- Respond promptly to emergencies with appropriate interventions
- Travel to various patient homes within the service area
Requirements
- Valid RN or LPN license for Delhi NCR region
- CPR/BLS certification
- Minimum of 2 years nursing experience; home care experience is highly desirable
- Strong clinical assessment abilities and critical thinking skills
- Excellent interpersonal and communication skills
- Ability to work independently and efficiently manage time
- Compassionate and patient-focused care approach
Preferred Qualifications
- Prior experience in home health or community nursing
- Familiarity with electronic medical records (EMR) systems
- Specialized training in geriatrics, chronic disease management, or wound care
- Multilingual abilities (if applicable)
- Possession of a valid driver’s license and reliable transport
Physical and Other Requirements
- Ability to lift up to 50 pounds
- Capacity to stand, walk, and bend for prolonged periods
- Willingness and ability to travel to multiple patient locations daily within a fixed zone
