Job Overview
Salinas Valley Post Acute is looking for a compassionate and organized individual to join their team as a full-time Social Services Assistant. This role involves working closely with the Social Services Director to advocate for residents, address their psychosocial needs, and assist with admission and discharge processes within a skilled nursing environment.
Compensation & Benefits
- Hourly wage between $20 and $21

Key Responsibilities
- Facilitate the admission and orientation process for residents and their families
- Coordinate safe and thorough discharge planning including referrals to community resources
- Maintain current and accurate medical records and social service documentation for residents
- Visit residents regularly to evaluate their emotional and psychosocial well-being
- Advocate for resident rights and report any concerns or grievances immediately to the Director
- Help organize resident council meetings and family support efforts
Qualifications
- High school diploma is required; Associate or Bachelor’s degree in Social Work or Psychology is preferred
- Experience in skilled nursing or long-term care facilities is highly valued
- Excellent written and verbal English communication skills; bilingual abilities are a strong advantage
- Compassionate, patient, and professional approach when engaging with elderly residents
- Familiarity with basic computer applications and electronic health record systems
