About the Role
Join Homerton Healthcare NHS Foundation Trust as a Health Visitor and contribute significantly to improving the health and wellbeing of children and families in the diverse City and Hackney areas of London. This full-time position involves working within a dynamic and expanding Health Visiting service collaborating with Public Health Staff Nurses and Community Nursery Nurses to deliver early intervention and support as part of the Healthy Child Programme.
Key Responsibilities
The Health Visitor will be allocated to one of the local Neighbourhoods, working closely with City of London and Hackney Children and Family Hubs to provide a comprehensive range of holistic services. The role includes acting as a named contact for GP practices, Early Years Settings, and Nurseries, managing vulnerable children through multi-disciplinary team meetings, and delivering health promotion activities and assessments using ASQ and ELIM questionnaires. Additionally, the Health Visitor will supervise the nursery nurse team's health promotion efforts and collaborate with local children's centres and parents to offer personalized advice supporting children's development.
