About the Role
Reema Health is searching for a dedicated Flex Community Guide to advance its mission of providing personalized, community-centered care across South Carolina. This licensed professional will operate in a hybrid work model, based in South Carolina, managing and supporting member caseloads while embracing the flexibility of a rapidly evolving startup environment.
Primary Responsibilities
- Engage a caseload of 60 to 100 members, including outreach via phone and community visits to enroll and support individuals facing complex health and social challenges.
- Conduct needs assessments to construct customized care plans based on social determinants of health and member goals.
- Establish trust and maintain ongoing communication using various formats, including electronic and in-person contacts, accommodating members' preferences.
- Coordinate referrals and support member participation in behavioral health, primary care, and other necessary services.
- Visit members in homes, treatment centers, or community locations to foster relationship building and meet identified needs.
- Efficiently organize workload to meet monthly objectives, documenting interactions and care plans accurately in the Reema App while maintaining confidentiality per HIPAA guidelines.
- Collaborate with team members across the company to exchange insights and adapt to evolving responsibilities.
- Ensure physical safety in the field through crisis planning, boundary management, and adherence to safety protocols.
Qualifications and Experience
- Bachelor's degree in Social Work, Human Services, Psychology, Health Sciences, or related fields. Relevant work experience can be substituted for education level.
- Minimum of three years of direct experience serving populations with complex health and social needs, with in-depth knowledge of South Carolina community resources.
- Strong communication skills and problem-solving abilities, capable of building meaningful, empathetic relationships.
- Proficient in using Google Workspace and other web-based digital tools for communication, documentation, and troubleshooting technical issues independently.
- Must be at least 18 years old with reliable transportation, valid driver's license, vehicle insurance, and a clean driving record.
Work Conditions and Schedule
- Position requires hybrid work primarily Monday through Friday, with occasional evening or weekend commitments.
- Travel required at 10-15% within a 25-mile radius.
- Ability to lift and occasionally carry up to 25 pounds.
- Compliance with federal and state privacy laws including HIPAA is mandatory.
- Successful candidates must clear background checks and drug screening pre-employment.

- Competitive annual base salary ranging between $50,000 and $54,000 (USD).
