Family Nurse Practitioner / Physician Assistant

Primary Responsibilities

• Deliver direct outpatient primary care by diagnosing, treating, and accurately documenting patient information following established protocols.
• Perform patient assessments, prescribe medications, and request laboratory tests or imaging studies as warranted.
• Oversee chronic disease management, assist patients in achieving self-management objectives, and regularly monitor health progress.
• Work collaboratively with multidisciplinary care teams, participate in case reviews, and serve as a mentor or preceptor for others.
• Engage in ongoing professional development, quality improvement efforts, and initiatives promoting patient-centered health home models.

Qualifications

• Current Basic Life Support (BLS) certification from the American Red Cross or American Heart Association.
• Valid full licensure to practice medicine granted by the North Carolina Board of Nursing or North Carolina Board of Medical Examiners.
• National certification as a Nurse Practitioner or Physician Assistant.

Preferred Experience

• At least one year of clinical experience.
• Experience serving underserved or rural patient populations.
• Familiarity with team-based care models such as Patient-Centered Medical Home (PCMH) or Patient-Centered Health Home (PCHH).
