Role Overview
As a Care Advisor, you will assume full responsibility for managing patient journeys from start to finish, going beyond traditional support functions. You will partner closely with patients to guide them through the healthcare system, facilitating effective care coordination, program execution, and supporting sustainable behavioral changes to achieve meaningful health outcomes.
Key Responsibilities
- Act as the main contact for patients throughout their care programs.
- Build trust-based relationships by understanding patients’ aspirations, requirements, and obstacles.
- Assist patients in locating healthcare providers, scheduling appointments, and accessing needed services.
- Coordinate medical referrals, follow-ups, and provider communications to ensure seamless care.
- Lead 8+ week structured care programs featuring scheduled milestones and touchpoints.
- Ensure adherence to care plans, medications, and next steps; identify and close care gaps.
- Encourage patients to develop sustainable habits and hold them accountable for follow-through.
- Monitor patient progress and escalate issues when necessary.
- Advocate for patients by clarifying care options and acting as a consistent partner within a fragmented healthcare system.
Candidate Profile
- Minimum of 2 years’ experience in healthcare, care navigation, or related support roles.
- Exceptional communication skills with ability to quickly build trust.
- Detail-oriented and highly organized.
- Adept at managing multiple patients and workflows concurrently.
- Proactive, resourceful, and focused on effective execution.
- Driven by achieving positive patient outcomes rather than just completing tasks.
Preferred Qualifications
- Experience working with Medicare populations.
- Background as Community Health Worker, Medical Assistant, health coach, or similar roles.
- Familiarity with care management, virtual care, or digital health solutions.
Success Indicators
- Consistent completion of planned patient touchpoints.
- High adherence rates to care plans and follow-up actions.
- Elevated patient engagement and satisfaction.
- Accurate and clear documentation supporting care delivery and billing processes.
Compensation and Benefits
- $40 per hour as a 1099 contractor.
