Position Overview
West London NHS Trust is inviting applications for a Band 5 Community Nurse specializing in Tissue Viability, based at the Heart of Hounslow Centre for Health on Bath Road. This full-time role operates Monday through Friday, from 9:00 AM to 5:00 PM. Candidates will provide expert nursing care focused on tissue viability, working collaboratively across multidisciplinary teams to ensure comprehensive patient management.
Key Responsibilities
- Provide skilled nursing interventions in tissue viability and support the Tissue Viability Nurses with patient-centered care planning, implementation, and evaluation.
- Lead and manage the Leg Ulcer clinics across various locations within Hounslow with guidance from Tissue Viability lead nurses.
- Collaborate closely with district nurses, primary care providers, rapid response teams, community matrons, and social services to deliver seamless care.
- Conduct clinic appointments and home visits, performing assessments and reviews to build and monitor individual care programs.
- Assist in service coordination, ensuring effective caseload and care management, including supervision duties during the absence of senior team members.
- Apply and maintain knowledge in wound care, leg ulcer management, and Doppler assessments.
