Role Summary

We are looking for a proactive Patient Coordinator Specialist to manage daily patient referrals and onboard new patients into our system. This role ensures referral accuracy, coordination with referral navigators, and delivering an exceptional patient experience as they begin using RxDiet services. The position is remote, requiring applicants to reside in the United States without sponsorship availability.

Key Responsibilities

• Oversee daily management of patient referrals and onboarding processes.
• Ensure accurate patient data entry and resolve referral discrepancies.
• Collaborate closely with referral navigators to coordinate patient care.
• Maintain communication both internally and externally to enhance patient experiences.

Qualifications

• Minimum two years of experience in healthcare settings.
• Direct prior experience working with healthcare patients and providers.
• Strong skills in Excel or Google Sheets for data handling.
• Highly detail-oriented with excellent organizational and time management abilities.
• Outstanding written and verbal communication skills.
• Self-motivated with an entrepreneurial mindset suitable for a startup environment.
• Genuine interest in food-as-medicine concepts, startups, and promoting community health.
