Role overview

The Coombe Hospital is seeking a Medical Social Worker to deliver direct social work support to patients, along with their partners and families, under the direction of the Principal Medical Social Worker.

Key duties

• Provide one-to-one medical social work assistance to the allocated patient caseload and their support network.
• Keep the Principal Medical Social Worker updated on important casework issues, including urgent cases, key hospital or department matters, and developments within the medical social work team.
• Offer counselling, practical information, and emotional support to hospital patients as needed.
• Maintain the records and statistical information required by the Medical Social Work Department.
• Support complex discharge planning through case management input.
