About Sabathani Community Center
Sabathani Community Center (SCC), established in 1966 as one of Minnesota’s earliest African American-founded nonprofits, serves a diverse community exceeding 50,000 residents in South Minneapolis, focusing on reducing social and economic disparities through youth programs, housing, health initiatives, and education.
About the Role
The Health Equity Department at SCC is dedicated to diminishing health disparities by providing culturally responsive and trauma-informed care through direct services, care coordination, and community education. We are seeking two Family Access Coordinators to lead and develop a newly established prevention services program for families involved or at risk within the child welfare system. The role centers on forming and managing referral relationships with Hennepin County Child Protection and other child welfare entities while ensuring compliance with state and federal prevention guidelines. The Coordinator will effectively integrate prevention programming into SCC’s broader family support services to deliver a cohesive support system for families.
Key Responsibilities
- Assist in launching a prevention program targeting families at risk or involved in child welfare.
- Build the organization’s capacity to deliver prevention services consistent with federal and state mandates.
- Complete necessary trainings for program readiness and maintain up-to-date knowledge on funding requests and regulation frameworks including Minnesota's Family First Prevention Services Act.
- Foster and strengthen referral pathways and partnerships with Hennepin County Child Protection and other tribal or county child welfare agencies.
- Implement coordinated referral systems among SCC’s programs and partners to facilitate timely access and engagement.
- Monitor referral and engagement trends to optimize service delivery.
- Integrate prevention services seamlessly with SCC’s existing family support offerings such as Supervised Visitation, housing assistance, maternal health initiatives, and mental health services.
- Collaborate with Family Support Specialists to align intake and service delivery processes.
- Support funding coordination efforts and sustainability planning, aiming to maximize federal FFPSA funding opportunities without disrupting current funding streams.
- Develop and share training resources and best practices for prevention services among staff and partners.
Qualifications
- Bachelor's degree in Social Work, Human Services, Public Administration, or related field mandatory; Master’s degree is preferred.
- Minimum of three years’ experience in child welfare, family support, prevention programming, or similar environments.
- Prefer experience in establishing partnerships with county or tribal child welfare agencies and coordinating referral systems across programs.
- Proven ability to work effectively with families at risk of or engaged in the child welfare system.
- In-depth understanding of child welfare systems, prevention programs, and family support methodologies.
- Excellent interpersonal skills to build and maintain effective external partnerships.
- Strong organizational capabilities to manage multiple programs, partners, and funding sources.
- Proficient verbal and written communication skills, including interactions with funders and partner organizations.
- Exercising sound judgment, maintaining confidentiality, and understanding professional boundaries are essential.
- Familiarity with Microsoft Outlook, Word, Excel; experience with data management or case tracking systems is beneficial.
- Valid Minnesota driver’s license and reliable transportation are preferred.
- Ability to pass a required criminal background check.
Compensation
The hourly pay range for this position is $29.00 to $32.21.
