Role Overview
The Intake Clerk is responsible for managing admission and discharge information for pharmacy customers in long-term care, assisted living, and institutional environments. The position requires careful handling of payer and demographic data for patients or residents, since this information directly affects pharmacy fulfillment and medication distribution. Accuracy and speed are both essential.
Location, Pay, and Schedule
This is a remote position based out of Vienna, VA. The pay range is $18 to $20 per hour. The working schedule is Tuesday through Friday from 12:00 PM to 8:30 PM EST, plus Saturday from 7:00 AM to 3:30 PM; Sunday and Monday are off.

Key Responsibilities
- Gather accurate patient details, including demographics, payer information, and insurance data from electronic client systems, eligibility portals, and internal customer databases.
- Enter information promptly into pharmacy fulfillment software so prescriptions can be processed and delivered on time.
- Stay in regular communication with coworkers, pharmacists, nursing facility teams, and insurance carriers.
- Sort and manage accounts according to patient demographic and insurance details.
- Handle discharge-related updates in the pharmacy system when needed.
- Update patient profiles correctly after discharge, including billing handling for medications filled on or after the discharge date.
- Check eligibility systems, then create and document patient and insurance records.
- Follow department procedures for documenting and reviewing insurance information.
- Escalate workflow-related concerns to the Intake Manager.
- Contact providers or facilities when additional information is needed to open accounts, verify eligibility, or share relevant updates.
- Support ongoing service, policy, and process improvements.
- Take on other tasks assigned by management to assist the department.
- Comply with Clarest’s Code of Conduct and compliance policies, and report any suspected legal or policy violations to a supervisor, Human Resources, or the Compliance Officer.
Qualifications
- High school diploma or equivalent is required.
- Working knowledge of Medicare Part D, Medicaid, and commercial insurance plans is required.
- Intermediate-level proficiency in Microsoft Outlook and Teams is required.
- Intermediate experience with Framework LTC and DocuTrack is preferred.
- Ability to protect confidential information, including HIPAA-regulated data.
