Role Overview
The organisation is looking for a committed and enthusiastic Nutritionist to support its Community Care team. In this role, you will contribute to preventive health and healthy ageing for seniors in Active Ageing Centres by providing nutrition education, dietary evaluation, and tailored nutrition support.
You will work alongside an integrated healthcare team that includes doctors, TCM physicians, nurses, and allied health professionals. Together, the team helps seniors stay healthier, manage long-term conditions, and improve day-to-day wellbeing.
What You Will Do
Health Promotion and Education
- Plan and run nutrition and wellness activities for older adults.
- Present talks and workshops on healthy eating, hydration, chronic disease prevention, food hygiene, weight control, and healthy ageing.
- Create learning materials and handouts for seniors and caregivers.
- Organise healthy eating campaigns and live cooking demonstrations.
- Contribute to broader public health and wellness initiatives in the community.
Nutrition Screening and Support
- Carry out nutrition screening and assessments to spot concerns such as malnutrition, frailty, sarcopenia, obesity, and other diet-related issues.
- Offer one-to-one nutrition counselling and practical dietary guidance.
- Build and apply nutrition care plans for seniors recovering from illness, surgery, or unintentional weight loss.
- Track client progress and update interventions when needed.
- Support seniors managing conditions such as diabetes, hypertension, hyperlipidaemia, kidney disease, and osteoporosis.
Team-Based Care
- Take part in case reviews and care planning with multidisciplinary healthcare colleagues.
- Work closely with the Integrated Health Team to deliver personalised and holistic senior care.
- Arrange referrals to suitable healthcare and support services where necessary.
Community Engagement
- Deliver nutrition services at Active Ageing Centres and other community outreach sites.
- Join community engagement efforts, workshops, and wellness events.
Working Hours
- Monday to Friday, 8:30am to 5:30pm.
- Some evenings or weekends may be needed for community programmes and workshops.
Additional Information
This position is based in Jurong East, West Region, Singapore and involves on-site work. It is offered on a contract basis.
