About the Role
This is an exciting opportunity for a senior mental health practitioner to join the Fylde Coast Home Treatment Team. The team provides comprehensive 7-day support to individuals experiencing mental health crises, aiming to prevent acute inpatient admissions by delivering high-quality care in a community setting. We are committed to a recovery model and effective risk management. The team is currently undergoing significant changes to enhance service delivery in collaboration with local services and across the Urgent Care Pathway.
We are seeking a dynamic, forward-thinking practitioner passionate about working with individuals experiencing acute mental health concerns and dedicated to a recovery-focused approach. The successful candidate will demonstrate strong teamwork and partnership capabilities with local services.
Key Responsibilities
- Provide effective and timely initial assessments for individuals experiencing mental ill health, creating appropriate pathways to avoid unnecessary escalation to crisis or A&E, thereby ensuring effective patient care and cost savings.
- Act as a crucial member of the multi-agency team, conducting initial telephone assessments and triage for individuals with suspected mental illness or vulnerability who might otherwise present at A&E or other places of safety.
- Conduct comprehensive Mental Health and Risk Assessments, delivering rapid, holistic assessments, care plans, and signposting for individuals presenting with urgent risk issues.
- Proficiently refer to other clinicians within the service and/or external services and agencies as required.
- Fulfill a "gatekeeping" function as outlined in the Operating Procedure.
- Provide managerial support to the team in the absence of the Team Manager.
