Role Overview

Ennoble Care is hiring an experienced full-time Patient Care Coordinator to work remotely and support patients in the Orlando, Florida area. This role helps maintain high-quality care delivery by coordinating between clinicians, staff, patients, caregivers, and families. The ideal candidate is compassionate, highly organized, and comfortable managing multiple care needs while keeping communication clear and consistent.

Key Responsibilities

  • Build and maintain individualized care plans, and carry out care management and coordination tasks using the company’s electronic medical record system.
  • Stay in regular contact with patients to support care coordination, reinforce program compliance, and help improve patient outcomes.
  • Record all patient and client communications accurately, whether they happen by phone or in writing.
  • Communicate patient emergencies or potentially life-threatening situations to the primary care clinician or appropriate delegate, and help ensure care plans remain complete and accurate.
  • Keep the team supervisor updated on any issues related to the care plan workflow or other observed concerns.
  • Work effectively across departments to support patients, families, and partner facilities.
  • Use secure email, phone systems, electronic medical records, and related technology with confidence.
  • Follow documentation standards, maintain daily work logs, escalate client concerns appropriately, and support internal communication best practices.
  • Provide a strong customer service experience reflected in positive feedback from patients and customers.
  • Contribute as a cooperative and constructive team member who supports colleagues and department goals.
  • Operate standard office tools such as a computer, telephone, copy machine, and similar equipment as needed.

Qualifications

  • Comfortable spending much of the day speaking with patients and other contacts by phone.
  • Compassionate, empathetic, and committed to delivering excellent service.
  • Able to capture detailed notes and complete tasks accurately and on time.
  • Capable of managing several patients and workstreams at once while staying organized and efficient.
  • Able to perform well in a fast-moving environment.
  • Available to work full-time, Monday through Friday, from 8:30 a.m. to 5:00 p.m.
  • Proficient with computers, including Outlook and other Microsoft Office applications.
  • Has a working knowledge of basic healthcare terminology, conditions, roles, and foundational care principles.
  • Must be able to pass a drug screening and background check.
  • Should demonstrate a positive attitude, adaptability, teamwork, and a willingness to keep learning new skills.

Compensation and Benefits

The starting pay is $17.00 to $18.00 per hour.

Full-time employees are eligible for medical, dental, vision, and supplemental benefits such as life insurance, short-term disability, long-term disability, flexible spending accounts for medical and dependent care, accident coverage, critical illness coverage, and hospital indemnity.

Additional full-time benefits include paid time off and paid office holidays.
