Role overview

The Selden Centre is a contemporary, purpose-built 10-bed inpatient assessment and intervention unit for people with learning disabilities. The service is focused on helping people move back into community-based living. It provides a safe, welcoming setting supported by a team of around 30 multidisciplinary healthcare professionals.

The team offers personalised support, including one-to-one therapeutic sessions, for men and women referred to the service due to escalating needs in the community, breakdown of a placement that needs reassessment, or transition from a more secure and/or higher-cost residential or hospital setting.

What you will do

In this Band 5 Staff Nurse role, you will work in line with the principle that people with learning disabilities have the same rights as everyone else. You will collaborate with service users, families and carers, referrers, and other service providers to identify the outcomes that need to be achieved.

You will also help assess needs and develop tailored interventions and care plans that build the individual’s skills. Working closely with partner agencies and care managers, you will support the identification of suitable future placements that match each person’s care and support needs after discharge.
