Role overview

As a Patient Care Coordinator, you will play an important role in delivering attentive service to patients and clients by handling medical goods and service referrals from start to finish. The position focuses on accurate processing, clear communication, and timely follow-up to ensure requests move smoothly through the workflow.

What you will do

  • Contact patients about new or in-progress referrals, verify demographic details, share status updates, and confirm delivery of requested goods or services.
  • Coordinate with vendor partners, claims adjusters, and nurse case managers to keep everyone informed on both new and active referrals.
  • Identify and secure vendor partners needed to fulfill each referral request.
  • Handle incoming and outgoing calls related to existing referrals, new referrals, and referrals owned by other team members when needed.
  • Prepare quotes for client approval or follow up on previously sent quotes to obtain written authorization for requested referrals, making sure each request is medically necessary and approved by the claims adjuster.
  • Check that all required billing details are complete for each referral, then route referrals into the billing process each day.
  • Process referrals in accordance with state rules and workers’ compensation guidelines.
  • Take on additional duties as assigned, support quality initiatives, and travel when required.

Qualifications

You should have a high school diploma or GED, along with at least two years of experience in healthcare network coordination and patient care scheduling, or an equivalent mix of education and experience.

Skills and strengths needed

The role calls for strong service orientation, sound judgment, and the ability to manage multiple priorities in a fast-paced environment. You should be comfortable using Microsoft Office, communicating clearly in both speaking and writing, and producing accurate written reports. Strong organization, collaboration, problem-solving, and analytical thinking are also important.

Compensation and benefits

The starting pay range disclosed for this jurisdiction is 17.00.
