Role overview
The Patient Care Coordinator will partner with the care team to communicate and document high-risk patient details for providers and practice teams. In this role, you will work alongside a multidisciplinary team to support coordinated patient care while factoring in each patient’s personal health and lifestyle goals.
Location requirement
- Applicants must be based in Arizona and able to travel to clinics in Phoenix and/or Mesa.
Operating responsibilities
- Reach out to high-risk patients, recently discharged patients, and others who need additional care management support to ensure PCP follow-up visits are arranged and care gaps are addressed.
- Work with care team members and outside vendors to support services such as durable medical equipment and home health care.
- Obtain medical records from physicians and healthcare systems to support continuity of care for primary care providers and interdisciplinary team members.
- Contact, screen, and escalate patients to nursing and social work staff based on individual needs.
- Support nursing and social work teams with patient follow-up and provide tools or resources as directed.
- Track stable patient groups as directed by nursing and social work staff.
- Work with patients to resolve open care gaps and ensure the medical record contains the necessary supporting documentation.
- Partner with local community resources to address patients’ physical and psychosocial needs.
- Collaborate with multidisciplinary colleagues to deliver strong patient care and better outcomes.
- Take part in special projects and initiatives as assigned by market leadership.
Required skills and strengths
The ideal candidate should be able to juggle multiple priorities, stay organized, adapt well in changing environments, and maintain a constructive, solution-focused mindset. Success in this role also requires comfort working with diverse populations, initiative, honesty, follow-through, quick issue recognition, and strong communication that builds trust.
Experience and technical background
- At least 2 years of experience as an LPN, state-equivalent licensed practical nurse, or medical assistant.
- Comfortable using technology, including Microsoft Office tools.
- Prior experience with electronic health records, including data entry, data analysis, and reporting.
