About the opportunity

ProCare Fresh Minds is seeking expressions of interest from qualified mental health professionals who are passionate about supporting people in their communities across Central Auckland. This is an opportunity to join a new primary care behavioural health workforce as a Health Improvement Practitioner, working at the centre of general practice care and helping people work toward their own wellbeing goals through short, focused interventions.

The role is part of Te Tumu Waiora, an innovative programme that is expanding access to behavioural health support within general practice clinics across Tāmaki Auckland. The work is relationship-based, community-focused, and particularly centred on improving outcomes for Māori, Pasifika, and young people.

What the role involves

As a Health Improvement Practitioner, you will provide accessible, evidence-informed brief talking therapy support to individuals and whānau from within the GP clinic. You will help enrolled patients manage concerns affecting wellbeing such as mood, sleep, anxiety, stress, relationships, or adjusting to a new diagnosis.

You will work as part of both a general practice team and a wider network of Fresh Minds Health Improvement Practitioners and Health Coaches, contributing to a friendly, fast-moving, patient-centred environment.

Key duties

  • Deliver short psychological interventions, with a strong emphasis on Focused Acceptance and Commitment Therapy (FACT), and facilitate evidence-based group programmes.
  • Provide consultation, education, and practical advice to the general practice team while helping identify unmet needs for patients and whānau.
  • Work with GPs and nurses to shape local intervention pathways that reflect the needs of each clinic and community.

Who should apply

Applicants must be fully registered mental health professionals. Suitable backgrounds include psychology, psychotherapy, mental health nursing, social work, occupational therapy, or counselling with DAPAANZ registration. You should have some experience or training in talking therapies such as ACT, FACT, or CBT.

The ideal person will be self-driven, comfortable working independently, and able to collaborate effectively within multi-disciplinary teams. Because the role operates across two busy general practice environments, you will need to be resilient, adaptable, highly organised, and comfortable building strong working relationships with several teams. Strong typing and IT ability is also important, as clinical notes must be entered promptly into multiple practice management systems. Cultural awareness and sensitivity when working with diverse communities is essential.

Location and work pattern

This opportunity is based onsite in Auckland, New Zealand, across Central Auckland general practice locations. The employer is open to a mix of permanent part-time and full-time vacancies with different FTE and hour options, so candidates are asked to specify their preferred location and working range. Applications will be retained for future opportunities if there is not an immediate match.
