Role Summary
The Clinical Documentation Specialist-Nurse is responsible for reviewing patient charts before appointments, identifying missing information, and helping ensure records are complete, accurate, and compliant. The position works closely with clinical and administrative teams and may also involve direct communication with patients to clarify health history or collect incomplete details.
Key Responsibilities
- Examine patient records before scheduled visits to confirm they are complete and accurate.
- Spot missing or incomplete items such as past medical history, lab or test results, and referral details.
- Work with providers, nurses, and support staff to obtain any required information.
- Confirm and maintain patient demographic, insurance, and clinical data in the electronic health record system.
- Support compliance with documentation standards, coding practices, and regulatory requirements.
- Reach out to patients when needed to collect missing details or clarify medical history.
Qualifications
- Must hold an active Licensed Practical Nurse (LPN) or Registered Nurse (RN) credential.
- At least 1 to 2 years of experience in a clinical environment is preferred, especially with medical record review and interpretation.
- Strong planning, prioritization, and time-management abilities.
- Clear verbal and written communication skills, along with strong people skills.
- Comfortable using electronic health record (EHR) platforms.
- Capable of working on your own as well as collaborating with a team.
- Working knowledge of medical terminology and clinical procedures is preferred.
- Basic Life Support (BLS) proficiency is required.
Additional Requirements
- Marijuana cards are not permitted under Act 593.
- No facial piercings are allowed.

This is an on-site full-time position based in Pocahontas, Arkansas.
