Role overview

The Clinical Documentation Integrity (CDI) Specialist reviews inpatient medical records to confirm that documentation is complete, accurate, and compliant. The role supports proper reporting of severity of illness, risk of mortality, quality measures, medical necessity, and reimbursement. The specialist works with physicians, coding teams, and other clinical stakeholders to strengthen documentation quality and protect DRG integrity.

Key duties

  • Review inpatient charts concurrently and/or after discharge to verify documentation accuracy, completeness, and compliance.
  • Maintain an accuracy level of 98%.
  • Spot opportunities to clarify diagnoses, procedures, POA indicators, severity of illness, risk adjustment, and clinical validation.
  • Send compliant physician queries when documentation is conflicting, incomplete, or too vague.
  • Work with coding professionals to support correct ICD-10-CM/PCS coding and MS-DRG/APR-DRG assignment.
  • Track documentation patterns tied to mortality, quality measures, hospital-acquired conditions, and patient safety indicators.
  • Perform clinical validation reviews for high-risk conditions such as sepsis, respiratory failure, encephalopathy, malnutrition, and acute kidney injury.
  • Meet productivity and quality expectations, including a 35–40% query rate, 20–25 chart reviews per day, and 98% accuracy.
  • Take part in multidisciplinary rounds, provider education, and documentation improvement efforts.
  • Help reduce denials by strengthening documentation integrity initiatives.
  • Stay aligned with CMS guidance, ICD-10 standards, AHA Coding Clinic updates, and internal compliance requirements.
  • Follow corporate policies, procedures, and security standards while carrying out assigned responsibilities.

Experience and qualification requirements

  • Either a valid Registered Nurse license with a bachelor’s degree or higher, plus 7 years of nursing experience and 5 years in an inpatient acute care setting.
  • Or a High School Diploma/GED plus 11 years of experience in an inpatient acute care setting.
  • Experience with audit practices, ICD-10 and PCS guidelines, quality initiatives, and MS-DRG/APR-DRG methodologies.
  • Practical clinical knowledge that can be applied to coding and documentation review.
  • CCDs, CCS, or CDIP experience, along with 7 years of clinical documentation/DRG validation experience.
  • Ability to travel up to 25%.
  • Knowledge of Solventum’s HIS software portfolio, such as 360 Encompass.

Preferred strengths

  • Background in telecommuting and working with EMRs and other electronic chart review tools.
  • Current chart auditing experience.
  • Familiarity with 360 Encompass or related Solventum HIS tools.
  • Strong command of Microsoft Word, Excel, PowerPoint, and other Office applications.
  • Excellent communication, presentation, analytical, and organizational abilities.

Work arrangement and location

This is a remote role based in the United States. Travel may be required up to 25% for domestic and/or international needs. Relocation assistance may be approved. Candidates must be legally authorized to work in the country of employment without employer sponsorship.

Onboarding note

New hires starting on or after October 1, 2025 will need to travel to a designated company location for in-person onboarding during the first days of employment. The company will arrange and pay for related travel in line with its travel policy.

Pay and benefits

The expected compensation range for US applicants is $96,000 to $132,000 annually, including base pay and variable incentive pay if eligible. Final compensation depends on factors such as experience, skills, training, and work location.
