Role overview

Senior Doc is seeking an accomplished Care Management Leader to guide and grow a team of roughly 50 Care Managers and Medical Assistants within a large Chronic Care Management (CCM) program. This role reports to the Chief Operating Officer and is responsible for day-to-day leadership, process improvement, performance tracking, and building a strong culture of ownership and high performance.

The right candidate will be a seasoned operations leader who enjoys working in a fast-moving, metrics-focused setting and is energized by developing motivated teams and improving patient and provider experiences.

Key responsibilities

  • Oversee a team of about 50 Care Managers and Medical Assistants, providing clear direction and support.
  • Hire, onboard, coach, mentor, and develop team members.
  • Promote a workplace culture rooted in accountability, teamwork, urgency, and continuous improvement.
  • Hold regular coaching discussions, performance evaluations, and career growth conversations.
  • Create and manage dashboards, scorecards, and reports to track productivity, CCM minutes, documentation quality, compliance, and overall operational performance.
  • Run daily team huddles centered on output, quality, and workload balance.
  • Track performance trends and remove obstacles that reduce efficiency.
  • Design repeatable workflows and scalable processes that strengthen consistency and results.
  • Review data to uncover improvement opportunities across operations.
  • Recognize strong performers and provide additional coaching to employees who need support.
  • Put performance improvement plans and corrective actions in place when needed.
  • Share operational findings and strategic recommendations with senior leadership.
  • Protect compliance with CCM documentation standards and payer requirements.
  • Maintain audit readiness across care management activities.
  • Balance productivity goals with strong patient and provider satisfaction.
  • Act as an escalation contact for complex patient or provider issues.
  • Work closely with clinical, operations, technology, and administrative teams to improve care delivery.
  • Assess and introduce technologies, workflows, and process enhancements that improve efficiency and patient outcomes.

Requirements

  • Bachelor’s degree in Healthcare Administration, Business Administration, Nursing, or a similar field.
  • At least 7 years of progressive leadership experience overseeing large, metrics-driven teams.
  • Minimum 3 years of experience leading people managers or sizable operational groups.
  • Background in building dashboards, KPIs, scorecards, and performance reports.
  • Strong analytical ability with a track record of turning data into operational improvements.
  • Hands-on experience with EHR platforms, care management systems, and reporting tools.
  • Excellent leadership, communication, coaching, and organizational capabilities.
  • Preferred: Master’s degree such as MBA, MHA, MSN, or an equivalent advanced degree.
  • Preferred: Experience in Chronic Care Management, Population Health, Value-Based Care, or broader care management operations.
  • Preferred: Clinical licensure such as RN, LCSW, or a comparable credential.
  • Preferred: Leadership experience in healthcare operations, patient engagement, customer service, enrollment, call center, hospitality, retail, or similarly high-volume environments.
  • Strong fit for someone who performs well in a fast-paced, results-oriented setting and enjoys developing strong teams.

Compensation
