About the Role

Pinnacle Home Care is seeking a Documentation Management Specialist to support its home health operations in Fort Myers Beach, Florida. The organization serves as Florida’s largest independent Medicare-certified home health provider and has spent more than 20 years delivering patient-focused care in the home.

What You’ll Do

  • Accompany care coordinators to referral partner offices, build working relationships, learn referral patterns, and manage each partner’s preferred document delivery process.
  • Visit referral offices on a regular basis to collect signatures on active or overdue orders.
  • Coordinate with referral partners to recover missing paperwork when a care coordinator is no longer assigned to the agency or branch.
  • Check that physician details are accurate on profiles and investigate and correct any profile errors.
  • Submit Smartsheet updates for changes to assigned referral sources such as physicians, facilities, hospitals, or other information needing revision.
  • Record daily follow-up activity for open orders, including date, action taken, and next follow-up date, and review the prior week’s work.
  • Support order escalation by working with the home office, referral partners, and care coordinators.
  • Attend meetings to share updates on outstanding F2F items, hand-carry documents, escalations, and plans for retrieving missing paperwork.
  • Prepare and print orders for each care coordinator and help map out a weekly signature-collection plan.
  • Send outstanding orders, F2F documents, or hand-carry packets to care coordinators when they are in a referral office and can obtain a signature during the visit.
  • Run weekly F2F and order reports and partner with care coordinators to keep orders complete within 14 days, or within the timeframe appropriate to the patient’s case.
  • Review potential write-off accounts with RDS and determine whether the account can be resolved or should be written off.
  • Communicate professionally across departments to close out pending orders.
  • Answer inquiries quickly and work toward timely resolution of concerns or discrepancies.
  • Follow HIPAA requirements across all accounts.
